Send a brief enquiry with your contact details, whether you'd prefer a home visit or hospital appointment (with your home address or preferred hospital), and who is referring: GP, consultant, self or insurer. No clinical details are collected on the public form. Submitting the form does not book an appointment.

Information gathering

Dr Wright's team will seek consent from the patient, whoever holds a Lasting Power of Attorney or from the next of kin where there's no LPA. They may also contact the patient, referring clinician, or patient's GP to gather background medical information.

Clinical review and suitability

Dr Wright reviews whether the outpatient service is suitable and, where relevant, will try to coordinate any tests or investigations around the appointment so that as much as possible can be addressed in one visit.

Make a Booking

If you have requested a home visit, Dr Wright's team will respond directly to agree a convenient time, confirm who will be present (it helps to have someone who knows the patient well in the room), and share parking details.

Hospital booking enquiries are passed to the relevant hospital outpatient booking team. They will contact you about appointment arrangements.

On the day

On the day, the clinic and hospital teams will coordinate the appointment around the patient's needs where possible. Any necessary tests or investigations will be arranged where clinically appropriate and feasible. If hospital transport may be needed, please let the team know in advance so this can be discussed with the hospital.

After the appointment

Dr Wright provides a written report and a clear management plan. Further tests, follow-up and recommendations are discussed and agreed with the patient. With the patient's consent, the report can be shared with the GP or wider care team.
